CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.

Operating out of 23 sites – including three outpatient centers located in Paramus, Fair Lawn, and Montclair, ten residential facilities, offices at three local hospitals, and seven community offices – our services are convenient to clients located throughout Northern New Jersey. We also provide educational programs on mental health-related topics to area schools and community groups.

Responsibilities:

Per Diem- Case Manager

  • Provide care coordination among a consumer’s health providers
  • Assure coordination of care transitions
  • Provide prompts for medication adherence, diet adherence, and activity adherence
  • Assure health team communications to maximize coordination of care
  • Maintain documentation of activities and services provided in accordance with regulatory requirements and agency standards
  • Assure outcome data elements are maintained timely and accurately so that health care impact can be monitored    
  • Facilitate and coordinate psycho-educational services for consumers and their family members on a variety of topics relating to wellness, health promotion, prevention, skill-building and related health and wellness/recovery topics
  • Offer community based mobile outreach services (including transportation of consumers)
  • Fulfill case management services, including coordinating or directly facilitating linkages to social services, treatment, specialty care, transportation services and so forth
  • Engage in supportive counseling and crisis intervention
  • Assess/evaluate consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborate with team and collaborative treatment providers to establish comprehensive bio-psycho-social assessments
  • Formulate consumer-centered care plans in collaboration with team members, containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives

Qualifications:

  • BSW or related mental health field
  • Two years experience working within a healthcare or behavioral health setting is a must
  • Possess a valid driver’s license, and must be insurable under Agency's automobile insurance policy
  • Excellent computer skills
