Department: Assisted OP Treatment Services
Location: Hasbrouck Heights, NJ

CarePlus New Jersey is a northern New Jersey provider of recovery-focused mental health, substance abuse, and family services that address the unique needs of individuals with a holistic approach to recovery and overall wellness.

Operating out of 23 sites – including three outpatient centers located in Paramus, Fair Lawn, and Montclair, ten residential facilities, offices at three local hospitals, and seven community offices – our services are convenient to clients located throughout Northern New Jersey. We also provide educational programs on mental health-related topics to area schools and community groups.

Responsibilities:

  • Responds to customer-defined crisis 24 hours per day, 365 days per year and provide crisis intervention and consultation to customers, their families, and others.
  • Contacts customers on a regular basis to assess risk level.  Assists costumers with ADL skills by providing necessary services (assistance with medication monitoring, linkage to community services and resources, personal care, money management, cleaning, shopping, vocational needs, substance abuse services, clothing, banking, etc.)
  • Facilitate all legal and court appointments
  • Facilitate psycho-educational groups for consumers on a variety of topics relating to wellness and recovery
  • Offer on-site as well as outreach services (including transport of consumers)
  • Fulfill case management services, including, but not limited to direct linkages to services, service coordination, treatment enrollment assistance, and so forth
  • Communicate observations and information about the consumer in the consumer’s service record and to team members
  • Engage in supportive counseling
  • Assess/evaluate consumers at intake/admissions, during ongoing recovery/service delivery and at discharge
  • Collaborate with treatment providers to establish comprehensive assessments, WRAP  and Individualized Recovery Plans
  • Formulates Recovery Plans containing relevant goals, measurable objectives (that serve as indicators for evaluating progress) and realistic time frames for achievement; performs reviews of progress and evaluates effectiveness of goals/objectives
  • Works with administrative staff to ensure that all services are documented based on billing and regulatory standards; collects information on consumers' insurance and reflects all demographic data
  • Participates in quality assurance activities and cross-functional action teams

Qualifications:

  • Master’s Degree in Social Work, Psychology, Criminal Justice or related field.
  • Knowledge of word processing software; spreadsheet software; internet software and electronic health records database software
  • Substance Abuse Credentials (e.g.: CADC) is preferred
  • Possess a valid driver's license, insurable under Agency's automobile insurance policy
